A Home Care Package (HCP) is funding provided by the Government that is given to elderly individuals that help them remain at home for as long as possible. We (such as My Aged Care) offer a higher level of support if you have more complex needs.
There are four levels of affordable aged care package each with a different level of funding, from Level 1, is the lowest amount that an individual can receive which is currently around $8,750 per year, Level 2 which is around $15,500. Level 3 is approximately $34,000 and Level 4, which is the highest level of care and funding, is approximately $51,500 per year. *These figures are rounded.

Our Home Care funds aren’t paid to you directly. Instead, you must choose an approved affordable package provider for home care to administer the funds on your behalf and support you with your care management. Carers & Companions can assist you with this and continue to provide Care Services

This step-by-step guide has been put together to guide you through the application process to obtain a affordable aged care package through the My Aged Care Government program.

By contacting My Aged Care on 1800 200 422. During the contact, they will assess that you meet all the government required for aged care funding.
An ACAT assessor will come and visit you at your home and will discuss with you your personal circumstances to assess and conclude what level of support you may need.
When your assessment has been completed, you will soon receive a letter advising you whether you have been approved to receive a home care package providers and then you will be placed in a priority queue and will be contact with a suitable package becomes available for you (this is based off how long you have been waiting for care in addition to your personal needs and circumstances)
You will receive a letter from My Aged Care confirming everything and the letter will contain a unique referral code. Once you receive this code, you may get in contact with Carers & Companions (1300 290 225) and we can assist you to kick start your plan.
